One of our nurses will assess your loved one's living situation and physical condition, in order to create an individual care plan tailored to your loved one's actual needs. Of course, life at this stage can change and often does; so, we will also ensure periodic check-ins with our highly qualified nurse to ensure the plan stays current and as helpful as possible.

Wondering what we can do for you?

Every client will receive an initial assessment by one of our nurses to create a personalized care plan specific to your loved one’s needs. The list below are examples of what that may include.


  • For clients bed-ridden, managing and turning out the bed linens, changing bed pans and/or assisting with adult briefs, helping client in and out of bed, if directed by care plan, etc.
  • Cooking nutritional meals that match your loved one's dietary needs and medical restrictions, as assessed by our nurse.
  • Assisting with mobility for those in need of wheel chairs, a side-by-side walking assist, help getting up from chairs or bed, etc.
  • Assisting your loved one with hygiene and cleanliness routines, such as bathing, getting hair done, using the restroom, getting dressed, etc.
  • Assisting with medications and ensuring they are taken on time and properly.
  • Assisting in managing negative behaviors (some examples: dissuading clients from wandering or acting out in a manner dangerous to their own well being).
  • Doing laundry
  • Giving your loved one a compassionate listening ear, including when client is talking about things that are not mentally sound or influenced by dementia, Alzheimer’s, etc.
  • Pre-hospice care or stay at home hospice care: staying by client bedside for as many hours as you need us and monitoring vitals, including managing shifts for up to 24/7. 
  • Cooking some meals.
  • Keeping a watchful eye over them as they sleep.
  • Be a kind and understanding ear, when your loved one speaks of life, past memories, or other heart-opening moments, whether the words make sense or not.
  • Keep you informed of how your loved one is doing or of any concerns or changes in his/her behavior.
